Eligibility Requirements

• Those over the age of 12 who have completed their 1st and 2nd vaccinations
• Three months or more have passed since the last vaccination

Booster Start Times

Since September 26, 2022, Hanamaki has begun using vaccines for the Omicron strain for those receiving boosters (3rd time or more being vaccinated).

  Appointments can be made for those who have received their Appointment Information (予約案内) form, which began shipping on November 1st (note that a pre-vaccination exam form and vaccination voucher was sent on October 21st).

• Those who have completed their last vaccination (2nd – 4th) in September:

IMPORTANT

Reception will begin at scheduled vaccination times. Prior to reception start, no waiting area is available at each location, so please arrive on or after the scheduled vaccination time. If you are late for your scheduled vaccination time, you may still be accepted before the day's closing time. Vaccinations may be held as late as 30 minutes after the last vaccination time. (Ex: if the last vaccination time is 4: 30pm, it may be until 5: 00pm) Those 65 years of age or older, or those with physical disability certificates (level 1 or 2), may use shared transportation provided by the city (even for individual vaccinations). For those who wish to use this service, please contact the city call center (0120-383-225) at least 10 days prior to the day of vaccination.

IMPORTANT

For those aged 12 – 17 getting their 4th vaccination, only the Pfizer vaccine is available. Other than a select few locations, you cannot make an appointment by directly calling one of the above medical institutions. Reception is held at the specified times to avoid crowding. You cannot enter a venue before the specified time. If you require assistance with wheelchairs, etc., or vaccination procedures, please contact the call center. The call center will contact your desired medical institution with your request. Depending on the date, different vaccines may be used even at the same medical institution. Be sure to check before making your reservation. Those 65 years of age or older, or those with physical disability certificates (level 1 or 2), may use shared transportation provided by the city. For those who wish to use this service, please contact the city call center (0120-383-225) at least 10 days prior to the day of vaccination.

Notes Regarding Those Aged 16 and Under

• A parent or guardian's signature is required on the pre-vaccination exam form.

【When attending mass vaccination】

• Junior high school students or younger must be accompanied by a parent, guardian, or their specified representative (an adult relative, etc., who is familiar with the health of the child to be vaccinated).

【When attending an individual appointment】

• Elementary school students or younger must be accompanied by a parent, guardian, or their specified representative (an adult relative, etc., who is familiar with the health of the child to be vaccinated).
• Junior high school students must be accompanied by a parent, guardian, or their specified representative (an adult relative, etc., who is familiar with the health of the child to be vaccinated). However, if this is not possible due to unavoidable circumstances, the parent or guardian must sign the pre-vaccination exam form and provide an emergency contact number (a phone number that can be used to contact the parent or guardian at the time of pre-examination or vaccination) in the Phone Number (電話番号) section.

City Provided Transportation

Those 65 years of age or older, or those with physical disability certificates (level 1 or 2), who have difficulty in securing a means of transportation may take a shuttle provided by the city to the vaccination location. Reservation is required, and can be made via the following:

1. If you make a vaccination appointment via City Call Center:
   Call the City Call Center at 0120-383-225 (JP language only) to make your vaccination appointment and reserve transportation at the time same time.
2. If you make a vaccination appointment online or via LINE app:
   Call the City Call Center at 0120-383-225 within the specified vaccination reservation period to reserve transportation.
