IMPORTANT
  • Those who have not yet received their 1st or 2nd vaccinations cannot receive the Omicron booster vaccine.
  • If you wish to receive additional vaccinations, please consider doing so within the year.
  • Note that for those receiving their 1st vaccination, Hanamaki provides the conventional Pfizer vaccine, as well as the Takeda vaccine (Novavax).
  • If you wish to be vaccinated with the conventional Moderna vaccine, please consider attending a vaccination appointment through the Prefecture.

※ After getting vaccinated for the first time, it is necessary to get the same vaccination a second time, after an interval of three weeks. 

Making an Appointment (1st & 2nd Vaccination)

If you have not yet received your 1st or 2nd vaccinations, please make an appointment to do so as soon as possible, via the city call center.

【About the Pre-Vaccination Exam Form】
As of December 1st, 2021, the pre-vaccination exam form has been changed. The new form can be downloaded from the Ministry of Health, Labor, and Welfare website, and is also available at vaccination venues.

Notes Regarding Those Aged 16 and Under

  • A parent or guardian's signature is required on the pre-vaccination exam form.

【When attending mass vaccination】

  • Those aged 15 years or younger must be accompanied by a parent, guardian, or their specified representative (an adult relative, etc., who is familiar with the health of the child to be vaccinated).

【When attending an individual appointment】

  • Elementary school students or younger must be accompanied by a parent, guardian, or their specified representative (an adult relative, etc., who is familiar with the health of the child to be vaccinated). However, if this is not possible due to unavoidable circumstances, the parent or guardian must sign the pre-vaccination exam form and provide an emergency contact number (a phone number that can be used to contact the parent or guardian at the time of pre-examination or vaccination) in the Phone Number (電話番号) section.
