  • NOTE: Children who turn 5 years old before receiving the first dose of a baby vaccine are eligible for the pediatric vaccine (for those aged 5 – 11 years old).

Type of Vaccine Used

The vaccine administered for those aged 5 – 11 years old is:

What to Bring to Your Appointment

  • Pre-vaccination exam form & vaccination voucher
  • Maternal Health Record Book (or vaccination certificate)
  • Health insurance card
  • Medication Record Book (for those taking medication)

When Attending Your Appointment

Children must be accompanied by a parent or guardian, and present a signed pre-vaccination exam form.

When receiving a vaccine, it is asked that everyone have the correct knowledge of both its prevention effectiveness, and risk of side effects. The parent or guardian is responsible for determining whether their child will be vaccinated or not. No vaccination will be administered without the consent of the parent or guardian. There should be no forceful vaccination of others or discriminatory treatment of those who are unvaccinated.

If you have any questions or concerns about vaccination, please consult your doctor.

Consult your doctor about dose intervals between Covid-19 vaccines and other vaccines. In general, other vaccines cannot be administered at the same time as, or two weeks prior or after, administering Covid-19 vaccines. Consult your doctor with additional questions or concerns, such as those regarding a child's underlying condition, as well.
